  • Acknowledges and fully understands that he/she will be engaging in activities that involve risk of serious injury, including permanent disability and death and sever social and economic losses which might result not only from their own actions, inactions, or negligence, but that of others, the rules of play, or the conditions of the premises or any equipment used. Further that there may be other risks not knows to him/her or not reasonably foreseeable at this time
  • CONCUSSION POLICY:If a coach, board member or referee suspects that a player may have a concussion, that player will be immediately removed from play. That player will not be allowed to return to ASDVNJ games, practices/pickup sessions, or tournaments until they provide written clearance from a licensed physician.
  • Assumes all foregoing risks and accepts personal responsibility for the damages following any injury, permanent disability or death
  • Acknowledges that although he/she is participating in a ASDVNJ program, he/she is NOT an official ASDVNJ member and is not insured as such and does not have insurance coverage from ASDVNJ
  • Releases, waives, discharges, and covenants not to sue the A.S DES VETERAN NEW JERSEY , their respective administrators, directors, agents, coaches, employees, and volunteers of the organization, other participants, sponsoring agencies, sponsors, advisors, and if applicable, owners and lessors of the premises used to conduct the event, all of which are hereinafter referred to as “releasees”, from demand, losses, or damages on account of injury, including death or damage to property, caused or alleged to be caused in whole or part by the negligence of the releasee or otherwise.
  • Releases, waives, discharges, and covenants not to sue the A.S DES VETERAN NEW JERSEY , their respective administrators, directors, agents, coaches, employees, and volunteers of the organization, other participants, sponsoring agencies, sponsors, advisors, and if applicable, owners and lessors of the premises used to conduct the event, all of which are hereinafter referred to as “releasees”, from demand, losses, or damages on account of injury, including death or damage to property, caused or alleged to be caused in whole or part by the negligence of the releasee or otherwise.
